We are a FEE-FOR-SERVICE Practice
We do not contract with any health insurance plans. Upon receiving exceptional care you will be provided with the necessary paperwork to submit a claim with your insurance company. Ultimately your insurance company will determine if our services are covered and to what extent.
We accept cash, checks, VISA, AMEX, Mastercard, Discover, HSA and FSA.
Cancellation Policy: Due to the large block of time allotted for initial visits, a $100 credit card deposit will be required at the time of booking. The deposit will go towards the cost of the initial visit. It is fully reimbursed if you cancel or reschedule greater than 2 business days prior to your appointment. Follow up visits also require a 2 business day cancellation/reschedule notice and may be subject to a $75 fee.
Parking is available in the rear parking lot with direct access to the office entrance. Suite JJ is located on the second floor of the business plaza. We are in the same business plaza as Mama Ricotta's Restaurant.
